For this assignment you will pick ONE of the following chronic disease states:
• COPD • CHF • Chronic kidney disease • Rheumatoid Arthritis • Parkinson’s Disease
Once you chose which disease process you would like to create your education brochure on. pick the top 3 medications or in other words the most common medications you would anticipate this patient to be prescribed based on the disease.
You will create this education utilizing the “drug card’ method you have used in previous modules to ensure you are gathering all the pertinent information to include in the brochure.
